Custom Order Form

If you need the order by a certain date, please let me know. If there is no specific date required, leave blank.

Do you have a specific size or style of lamp in mind? For example:

• 750 ml wine bottle
• square shaped bottle of any size
• bottle about 12 inches tall.
• Candle holder
• Ikea Grono Lamp

Or if you are unsure that's okay, just write - please advise me - we can discuss that later depending on what you want on the lamp.

What sort of design do you have in mind? Describe as much as you can. For example, sunflowers or a butterfly; what colours you want, etc.

If lamp needs to be shipped, where will it be shipped to? (I only need country, province/state, and post/zip code at this stage just to obtain an idea of shipping costs and time for delivery to the destination).

If you have seen a lamp I have created previously that you particularly like, you can add links to photos on my site or social media pages to show me.

Copyright Terms: It is perfectly acceptable to show me photos in order to help me gain an understanding of your vision. This is helpful for me to get a better idea of colours or styles you like. However, please do not present me with a photo of another artist's work and ask me to copy it, the answer will be no. Any photos you present to me must either be purely for inspirational purposes - to help express your preferences, or be owned by you. I will not reproduce any logos/drawings etc that are copyright to a third party without express permission to do so. If you are the copyright holder you must provide permission for me to replicate the design in writing (email) prior to the work commencing.
